The two levers on almost every hospital bill: duplicate or unbundled codes, and the discount billing offices give for paying promptly or in one payment. We also check the bill against your EOB and ask about financial assistance when you qualify.

Insurance appeal

A denial is a letter. An appeal is a better letter.

  1. 1Read the denial code and the appeal deadline. Most plans give 180 days; we file well inside it.
  2. 2Pull the plan's own medical policy for the service and the notes from the prescribing clinician, under your HIPAA authorization.
  3. 3Write the appeal to the policy's criteria, attach the records, and send it by the channel the plan honors. Keep the fax confirmation.
  4. 4Call at day 15 and day 30. If the internal appeal fails, request the external review you are entitled to.

What counts as one bill, one appeal, or one call?

One bill is one statement from one billing office for one patient. A hospital visit that produces a facility bill and a separate physician-group bill counts as two, because each is negotiated with a different office. Follow-up on the same bill, however long it takes, is part of that one bill.

One appeal is one denied claim or prior-authorization decision, through every internal appeal level the plan offers. One call is one goal with one organization, even if reaching it takes several attempts or callbacks. Cancellations are unlimited.

How long does it take?

First substantive action, meaning a call placed, a records request sent, or an appeal drafted: within 3 business days for medical bills, 5 for insurance appeals, 1 for phone calls, and 2 for cancellations. Most calls and cancellations are finished within a week.

The rest depends on the other side. Bill negotiations usually take two to six weeks because hospitals take time to produce itemized statements. Insurers are allowed 30 to 60 days to decide an appeal. You can watch progress in the app the whole time.

What do I need to give you?

A photo or PDF of the bill, denial letter, or account statement; your insurance card if the request involves a plan; and a sentence or two on what you want. For calls and cancellations, the account holder name, the account or phone number if you have it, and times you can be reached if the company insists on a callback.

For medical bills and appeals you also e-sign a HIPAA authorization in the app, and for calls and cancellations a short letter of authorization. Each takes under a minute.

How is my health information handled?

We act under the HIPAA authorization you sign, which lets providers and insurers release billing records to us for negotiating and appealing. Documents you upload are stored encrypted in private storage and are visible only to the agent working your request and the operations staff supervising them. Nothing from a bill or medical record is used for marketing or shared with advertising platforms. Details are in the Privacy Policy.

Can you act for my mom or dad?

Yes. Add them as a person on your account. If you hold power of attorney or are their health care proxy, you sign the authorizations yourself and upload the document if a company asks for it. Otherwise your parent signs from a link we send them; it takes about a minute on a phone. Some companies will only speak to the account holder, in which case we schedule a short three-way call so they can confirm and then step away.

What if I want to cancel Offload?

Cancel from your account settings any time; no call, no retention desk. Your plan stays active until the end of the period you already paid for, and open requests are finished. There are no cancellation fees.

What is your refund policy?

The $29.00 setup fee on monthly plans is not refundable. Subscription fees are not refunded for partial periods, but you keep the service through the period you paid for. Extra bills, appeals, or calls you buy one at a time are refundable within 30 days if we have not started work on them. Full terms are in the Terms of Service.

Who actually does the work?

Trained agents on our own team, working from written playbooks for each type of request, under United States-based operations management that reviews outcomes and handles anything unusual. Agents cannot move money, open accounts, or change account ownership. Offload is operated by Lazarus LLC, a United States company.
